Outpatient care in Minnesota
MINNESOTA
Blue River Behavioral Health can check your insurance before you commit to anything. Verify insurance for rehab by filling out one short form, and our admissions team will take it from there. We accept all insurance plans, including out-of-network benefits, across all four levels of our Minnesota outpatient program. Our program serves adults 18 and older who are managing substance use, a mental health condition, or both. A family member can send the form for you.
Your plan details come straight to us, and we check your benefits with your insurer. The form never asks for a payment card, and it doesn't enroll you in a program. After you hit submit, the rest is on us. You get a summary of your benefits for outpatient services, written in plain language with the billing codes left out. Questions go in the box at the bottom of the form.
Free and Confidential
Fill out the short form with your insurance details. Our admissions team contacts your insurer, then reaches out to you with the results.
Your information is kept confidential and used only to check your benefits. We do not sell or share it.
A family member can submit the form for a loved one, using that person's insurance details. If your loved one holds the plan, ask for a photo of the card, since it lists the member ID and the insurer's name. Insurers limit what they share with anyone but the member, so your loved one may need to join the call. Treatment starts only after the adult signs their own consent, which is step two of our admission process. You can still gather the details and fill out the form together.

Insurance and Coverage
Blue River accepts all insurance plans, including out-of-network benefits. Cost questions keep many adults from calling. We answer those first.
Share your plan details, and our admissions team contacts your insurer. Then we explain what your plan covers in plain language. Nothing you share commits you to anything.
No obligation. We never quote costs before your benefits are checked.
The Blue River admissions team contacts your insurer first, then follows up with you. Insurance for addiction treatment in Minnesota varies by insurer and by plan. We will let you know what your plan covers and any limits we find. A benefits check gives you time to weigh your options before you choose a level of care.
If you decide to move forward, pre-certification and the medical necessity review come first. Pre-certification means your insurer approves a service before it starts. The medical necessity review is the insurer's check that a level of care fits your needs. We complete both with your insurer, together. Then you will move through consent, a needs assessment, your treatment plan, and a psychiatric evaluation.
Out-of-network rehab benefits let you see a provider outside your insurer’s contracted network. Plans with these benefits often pay a smaller share for outside providers. Reimbursement amounts vary by plan. Some plans include out-of-network coverage while others do not. Submit the form even if your insurer’s directory doesn’t list us, because we work with many out-of-network plans. We check whether your plan includes behavioral health benefits and whether it requires approval before care starts.
Checking your coverage is a practical first step toward help with substance use, a mental health condition, or both. Complete the form to verify insurance for rehab with Blue River Behavioral Health, and our admissions team will explain your plan's coverage. Insurance rules are confusing, so we go through yours line by line. Contact us today with any questions about the form or your benefits.
Confidential. No obligation.
These answers cover coverage questions the form itself doesn't settle. We can answer anything not listed here.
(612) 614-0964Your plan pays for virtual IOP and virtual OP only if it lists telehealth benefits for behavioral health. Virtual benefits go through the same review as the in-person levels. A plan that caps telehealth sessions changes the math, so you hear about any cap before you start.
HMO plans can require a referral from your primary care provider before they cover specialty services. PPO plans generally do not. Enter your plan type on the form, and we will tell you which rule applies to you before your first session.
No, because a benefits check isn't an insurance claim and nothing gets billed. Once you start care, claims generate explanation-of-benefits statements, which the policyholder on a shared plan may receive. Federal privacy law covers your health details, and we release nothing without your written consent unless the law requires it.
Yes. Insurers can approve care in set blocks and review progress before approving the next block. Approval for the first block does not promise the second, so verification includes how your plan handles those reviews.
Send the form again with your new details, and we run the check from scratch. A benefits check is a snapshot of your plan on the day we run it. A new job or a new plan year can reset your deductible and change which services are covered.
